AGLP Treatment Guidelines

Guidelines for Residency programs care and support of trans, gender diverse, and queer patients

During interview season trans, non-binary, gender diverse medical students have been keenly aware of how ill-prepared residency programs are to welcome them to their programs. In addition to the anxiety already rife in the process of residency application there is the additional anxiety of needing to obtain essential information but being unsure if in outing oneself as trans or non-binary it might prevent them from matching.  

Because of this obvious disconnect between the desire to provide a welcome environment and the ability to do so, the Trans/Non-Binary Commitee of the AGLP: Association of LGBTQ Psychiatrists would like to present some recommendations for residency programs to support their incoming residents.

By establishing this groundwork within the residency program, each resident and attending will be able to provide more inclusive care to trans, gender diverse, and queer patients as well.    

We encourage programs to do the following:

Encourage all program staff, residents, and interviewers to include pronouns in Zoom names and email signatures

Establish clear policies and protection around names/ pronouns (ID badges, email, etc), and explicitly state those policies on your website

Establish clear policies around medical coverage and medical leave for gender-affirming medical and surgical care and associated medical leave, and explicitly state those policies on your website

Establish gender neutral bathrooms and locker room facilities and publish a map of those facilities on your program website

Establish clear policies around bias/harassment reporting, and explicitly state those policies on your website

Work with collaborative training sites to ensure consistency of these policies

We are including the position statement of the American Psychiatric Association for evidence. We also encourage you to further explore the resources available in the Diversity and Inclusion Toolkit provided by the Association of Program Directors in Surgery in June 2020.

AACAP Practice Parameter on LGBT Sexual Orientation, Gender Nonconformity, and Gender Discordance in Children and Adolescents 

Children and adolescents who are growing up gay, lesbian, bisexual, gender nonconforming, or gender discordant experience unique developmental challenges. They are at risk for certain mental health problems, many of which are significantly correlated with stigma and prejudice. Mental health professionals have an important role to play in fostering healthy development in this population. Influences on sexual orientation, gender nonconformity, and gender discordance, and their developmental relationships to each other, are reviewed. Practice principles and related issues of cultural competence, research needs, and ethics are discussed.
